On behalf of PT General Energy Bali, we are pleased to invite a Qualified Hospital to participate in our Medical Check Up for All Employee of PT General Energy Bali Tender/Enquiry. In order to adhere to the principles of openness, fairness and impartiality in competition and ensure the relevant work of Medical Check-up for All Employees services for the 3×142MW coal-fired power plant in Bali, Indonesia, PT General Energy Bali adopts the inquiry for proposal method to select the best supplier. The procurement matters and related requirements are hereby explained as follows:

2. Working location:

Medical check-up services shall, in principle, be conducted on-site at PLTU CELUKAN BAWANG (DESA CELUKAN BAWANG, KEC. GEROKGAK, KAB. BULELENG–BALI 81155) in a centralized manner.

The supplier shall have the capability to organize large-scale on-site medical examinations and shall provide necessary mobile medical examination equipment and medical personnel to ensure that routine medical check-up items can be completed on-site. This is to meet the centralized health examination requirements of plant employees, minimize off-site travel, and ensure that normal power plant operations are not disrupted.

3. Period of Service:

June 2026 to August 2026.

4. Scope of Inquiry:

Medical Check-up for All Employees services. The scope of work is detailed in the technical specification document.

5. Qualification Requirements:

1. An independent enterprise legal person or other organization legally registered in Indonesia with independent capacity to bear civil liability and the right to independently conclude contracts.
2. It is not in the process of being ordered to cease production, suspend business or enter into bankruptcy proceedings, and its assets are not restructured, taken over or frozen.
3. Hold a valid license to operate a healthcare facility issued by the Indonesian Ministry of Health or the local One-Stop Integrated Service Investment Agency (DPMPTSP).
4. It must have been officially accredited by an Indonesian national healthcare accreditation body (such as KARS, LAFKESPRI, etc.), and the certificate must be valid.
5. Have the same or similar project contract performance or have issued similar project completion documents. Including a photocopy of the contract (first page, reflecting the scope of service, signature and seal page, etc.).

6. Request for quotation:

1. Suppliers must submit unit prices and estimated total prices based on the estimated number of health check-up participants provided by the purchasing entity.
2. During the contract period, payments will be settled based on the actual number of participants who undergo health check-up.
3. The Indonesian rupiah (IDR) shall be the sole currency used for pricing and quoting in this procurement.
4. The response document is generally divided into three parts: technical part, commercial part (Company qualifications, business license, list of service achievements, etc.) and quotation part Quotation summary sheet. The response document should be uploaded before the deadline for submitting quotations.
5. The deadline for submitting quotations shall be the time specified in the inquiry email.
6. Suppliers shall quote in the format of the attached table. Do not modify the format when quoting. Fill in the data in the painted area. The total price and other data will be generated automatically.

7. Submission Deadline:

All submissions shall be sent through the procurement portal no later than 15:00 (WITA) on June 29th, 2026, kindly combine all together a ZIP File and submit to GEB Procurement Portal.
